    Male pelvic pain is complex - there so many different areas to evaluate and clear. Often it can be overwhelming and we jump straight to breathing and the pelvic floor.

    💡 The spermatic cord.

    This structure runs along the inguinal canal, carrying essential nerves and blood vessels that impact function and pain.

    Here’s why it matters:
    👉 Many male pelvic pain patients have tenderness, inflammation, or tightness in the spermatic cord.
    👉 Focusing only on the pelvic floor misses a key component of the pain cycle.
    👉 Treating the spermatic cord early can lead to faster symptom resolution.

    If you're treating penile, testicular, or scrotal pain, this is something you should be assessing and treating within the first few visits.

  • Pudendal Neuralgia: A Missing Piece

    Pudendal neuralgia is one of the most intimidating diagnoses we treat.

    It’s complex, frustrating, and often misunderstood—even among pelvic health providers.

    In this Missing Piece episode from the Rising Vault, we’re breaking down a key factor that’s often overlooked in pudendal neuralgia treatment:

    💡 The posterior cutaneous femoral nerve.

    It’s easy to hyperfocus on the pudendal nerve itself, but the reality is:
    👉 It’s almost impossible for pudendal neuralgia to only involve the pudendal nerve.
    👉 This is why nerve blocks often fail to relieve all symptoms.
    👉 This is why symptoms frequently extend beyond the pudendal nerve’s pathway.

    If we don’t consider the posterior cutaneous femoral nerve, we may be missing a major driver of symptoms—and limiting our ability to help these complex patients.
